An individual met all the targets that were set as part of the capability process, but now that the monitoring has stopped, their performance has become unacceptable again. What should the line manager do?

If the capability procedure is used and the employee improves but this improvement is not sustained during a 12 month period, line managers can revert back to the stage that the employee was previously at in order to manage the employee's performance appropriately.
