You should use the Agency Request Form to gain approval to recruit an Agency Worker/Contractor attracting payment of £220 per day or more.
You should have already had discussions within your Directorate with the relevant people who will need to approve this request before submitting.
Requests for Agency Workers (off payroll workers) should be used only in exceptional circumstances and only for genuine short term requirements where all other avenues have been exhausted. Approval of any requests will need to comply with our requirements in line with Department of Health Recruitment Restrictions.
Once submitted the details will automatically go for approval to the relevant people;
- Where resource is allocated to a Change Programme - your request will be sent to the relevant Director, Finance Approver and Budget Manager for approval.
- Where resource is not allocated to a Change Programme - your request will be sent to the relevant Budget Manager for approval
